Focusing on what you want to achieve, Support Coordination helps people with disability to understand and manage their NDIS plan independently, giving you greater choice and control.

 
What is support co-ordination?

Support Coordination is a service offered under the National Disability Insurance Scheme (NDIS) to help participants effectively navigate and utilize their NDIS plans. Support Coordination aims to assist participants in identifying and accessing the supports and services outlined in their plan.

Identify

Identify a range of suitable options for you in the NDIS marketplace.

Track

Keep track of your plan to ensure your NDIS funds are used effectively.

Coordinate

Coordinate Service Agreements and liaise with service providers.

Report

Provide the NDIA progress reports and prepare for your plan review meetings.

What we offer?

  • Plan Implementation- Our Support Coordinators work with participants to understand their NDIS plans and help them implement the supports and services outlined in the plan.
  • Connecting with Service Providers- Our Support Coordinators assist participants in finding and connecting with suitable service providers who can meet their needs and goals.
  • Capacity Building- Our Support Coordinators focus on building the participant’s skills, knowledge, and confidence to independently manage their NDIS plan over time.
  • Supporting Decision Making- Our Support Coordinators provide guidance and information to help participants make informed decisions about their supports and services.
  • Problem Solving and Advocacy- Our Support Coordinators help participants resolve any issues or challenges that may arise during the implementation of their plan. We also advocate on behalf of participants to ensure their rights and needs are met.
  • Monitoring and Review- Our Support Coordinators regularly monitor the participant’s progress and review their plan to ensure it remains effective and aligned with their changing needs.

How can help you?

To get the most out of your plan, you first need to understand the ins and outs. Support Coordination will guide you through each section, explaining what it means for you and assist you in aligning your plan with your NDIS goals.

Support Coordination aims to build your skills and confidence to independently manage your NDIS supports. Support Coordinators provide guidance and trusted advice so you can make confident, educated decisions about your supports.

When visiting a foreign place, a local is always the best guide. We’ve taken the same approach with our Support Coordination service. We have Support Coordinators based in each region so you have the best support local to you.

A Support Coordinator will endeavour to find supports that cater to your beliefs, languages, customs, rituals, behaviours, faith/religion, food and attitude. In fact, we even have Support Coordinators that speak in different languages.

Have a question about Support Coordination Services?
Support Coordination is not available to all NDIS participants; when you request Support Coordination funding, the NDIS will assess each person on a case-by-case basis to determine whether your individual circumstances meet the criteria for eligibility or not.
 

To assist you in understanding eligibility for Support Coordination below is an outline of circumstances where NDIS Participants may meet eligibility for Support Coordination funding.

Significant life change

A significant life change is an event or occurrence that has a major impact on your ability to live an independent and meaningful life, such as:

  • Moving to a new regional area.
  • Transitioning to short-term accommodation or shared independent living.
  • Leaving school and going to a new day program.
  • Exploring accommodation options or at risk of being homeless.
  • At the risk of being removed from a day program or kicked out of employment.

No existing informal supports

If you don’t have an informal support network, such as family or friends with the time or knowledge to help you find and connect with disability services, or if you have ageing parents or carers that no longer have the capacity to support you with the NDIS Plan. The NDIS may provide funding for a Support Coordinator to assist you.

Culturally and Linguistically Diverse (CALD)

Ream Community Services has a range of bilingual and bicultural Support Coordinators for CALD NDIS participants where English is not your first language, or you have limited knowledge of the NDIS and what disability supports and services are available to you.

Other

Other examples may include:

  • Families with multiple disabilities in the family who do not have the capacity to manage and coordinate several NDIS Plans (such as a parent with a disability or two children with disabilities).
  • Ageing parents who no longer have the capacity to support you to maintain your daily routines such as meal preparation, assistance to get ready for the day, or transport to and from your supports.
  • There is a high risk of harmful behaviour, mental health, or justice system involvement that may require urgent intervention and support.

Capacity building is a support that enables you to build your independence skills and capacity so as to reduce reliance on funded supports. You can visit our Services page and find out more about our capacity building supports.

Case Management is similar to Support Coordination in that it assists you to link with providers. Support Coordination is different to Case Management in that it is more about building your capacity, supporting you to become more independent.

The LAC program is a time limited support that works with the NDIA to build your plan and to start you on your NDIS journey.
 

The Support Coordination role is a funded support that will appear on your plan as a Capacity Building Support. The amount of time your coordinator has to work with you will vary for each participant, depending on the budgeted amount. Your support coordinator will work more closely with you to assist you to use your plan to meet your needs and to navigate the NDIS systems.

A Plan Manager is responsible for the financial/administrative side of a participant’s NDIS Plan. For example, the Plan Manager is responsible for organising payments, processing claims and invoices and tracking budgets. If you require plan management services, your Support Coordinator can help you to arrange this.
